Memorial Hermann Credentialing

A simple, straightforward approach
At MHMD, we work hard to ensure that our plan members receive the finest medical care possible. Through the MHMD credentialing process, we evaluate the qualifications of practitioners who provide care to our members and make sure they meet the highest standards set forth by accrediting agencies. Before any practitioner can be accepted for participation in the MHMD network, our Healthcare System or its affiliates, they must complete the credentialing process.

We know that credentialing can be arduous and time consuming for busy practitioners. At MHMD, we have been delegated responsibility for credentialing by each of our contracted health plans. This means that you only have to apply and be credentialed once including just one office site survey per year!

Membership/Credentialing Fees

Payment is required for MHMD Membership and for Memorial Hermann Credentialing. Fees may be payable by Check, or Money Order. Please make checks payable to requested facilities listed in the fee schedules below. Coming soon is the Electronic Practitioner Online Payment tool (ePOP) to make all payments online.
